About us: Release of Information is located within the Medical Records department, which is located on the first floor adjacent to the Mini Base Exchange and is open Monday through Friday from 7:30 a.m. to 4 p.m. While information contained in the medical record is considered confidential and personal, all medical records are the property of the United States government and must be maintained in the military treatment facility. The original hardcopy records will be mailed to the nearest military treatment facility if a patient PCS or move to another location. Patients can fill out a DD Form 2870, Authorization for Disclosure of Medical or Dental Information, at the Medical Records window. The request will be fulfilled within 20 duty days. All requests for records will be provide on a compact dis, unless paper records are specifically requested. The Privacy Act of 1974 states that medical records will not be released to any individual other than the adult to whom it belongs, or to the parent in the case of a minor (child under the age of 18). Each adult 18 years of age and older must complete a separate request for copies, unless an individual has granted authorization to another individual such as a spouse or a parent. If a patient has reached his or her 14 birthday, only the patient may authorize disclosure of information relating to sexual disease, birth control, drug, or alcohol abuse. If a patient has reached his or her 13 birthday, only the patient may authorize disclosure relating to mental illness. When authorized by the patients, medical record copies of retirees and their dependents will be released to themselves for personal use, to the civilian physicians for further medical care, to insurance agencies, and to their legal representatives via CD. Active duty personnel, upon separation or retirement, are entitled to one free copy of their medical records. Medical record copies will be furnished via CD. It is recommended members request a copy six months prior to final out-processing, when possible.
